No medication can be given without this form. The form can be faxed to 904-547-4165.<\/span><\/li><li><span class=\"elementor-icon-list-text\">All medications should be delivered by a parent\/guardian to the school in the original RX container with the child\u2019s name, and dose of medication on the bottle. All over the counter medications must be unopened and in original packaging.<\/span><\/li><li><span class=\"elementor-icon-list-text\">The FIRST dosage of any new medication will not be administered during school hours because of the possibility of an adverse reaction.<\/span><\/li><li><span class=\"elementor-icon-list-text\">If your child needs to carry an inhaler with them, there is a place on the medication form that the physician and parent must sign to authorize this.
